Descending Aortic Tearing due to Blunt Chest Trauma


Abstract
Acute traumatic rupture of the thoracic aorta resulting from blunt chest trauma is a highly lethal lesion requiring prompt diagnosis and immediate surgical correction. Commonly associated multiple injuries of other organs are relative contraindication to use of cardiopulmonary bypass or other complicated procedures in these critical patients.
I report here 2 cases of tearing of descending thoracic aorta at just below the left subclavian artery by automobile accident. They were treated by simple aortic cross clamping and repair with sutureless intraluminal graft in order to minimize ischemic time and morbidity. Aortic clamp time of the cases were 19 minL and 38 min. respectively. Postoperative course was uneventful in case 1. But in case 2, longer clamp time was required because of difficulties in insertion of proximal spool and hemiparesis was noted postoperatively.
Simple cross clamping of aorta and repair with intraluminal graft would simplify the procedure and meticulous preoperative planning and sufficient dissection of the aorta during surgery might improve the result.
